1/ Name, address and phone number of shipper

2/ Name, address and contact phone number of consignee

3/ Measurements of the animal in a standing position
a/ From ground to top of head
b/ Nose to bottom
c/ The width of your animal
d/ Inside front leg (from the ground to the chest)

4/ Breed of your animal

5/ Destination and approximate date of departure
